What is VO2 Max Training (Zone 5 Intervals)?

Peter Attia calls VO2 max the single greatest predictor of lifespan. His protocol is the 4x4 method: 4 minutes of near-maximal effort followed by 4 minutes of recovery, repeated 4-6 times. The goal is consistent near-maximal effort, not a perfect heart rate zone. By the third minute of each interval, you should be in significant discomfort.

Is there evidence for it?

EVIDENCEstrong — low VO2 max is one of the strongest predictors of all-cause mortality; improving from the bottom 25th percentile to above-average reduces mortality risk by ~5x (more than quitting smoking)

How do you do VO2 Max Training (Zone 5 Intervals)?

  1. 1.Perform one 30-minute VO2 max session per week using the 4x4 protocol: 4 min at 90-95% max effort, 4 min easy recovery, repeat 4-6 times
  2. 2.Choose any modality: cycling, rowing, running, swimming, or stair climbing — consistency matters more than the specific exercise
  3. 3.Use RPE (rate of perceived exertion) as primary intensity guide: should feel 'pretty easy' at minute 1, 'significant pain' by minute 3
  4. 4.Warm up for 5-10 minutes before intervals and cool down for 5 minutes after
  5. 5.Track progress monthly: aim to increase total work output per session over time; test VO2 max annually if possible
