What is Inspiratory Muscle Training (IMT)?

Resistance training for the breathing muscles (primarily the diaphragm and external intercostals) using a device that creates inspiratory load. Like 'dumbbells for the diaphragm.' Research shows it lowers blood pressure (by 9 mmHg systolic), improves exercise capacity, and reduces breathlessness. Protocols use 30-80% of maximum inspiratory pressure (MIP). Only takes 5-10 minutes per day. Devices include POWERbreathe, Threshold IMT, and SpiroTiger.

How do you do Inspiratory Muscle Training (IMT)?

  1. 1.Obtain an IMT device (POWERbreathe, Threshold IMT, or Airofit) and establish your baseline maximum inspiratory pressure (MIP) — breathe out fully, then inhale as hard as possible through the device to measure
  2. 2.Set the device resistance to 30% of your MIP for the first week; sit upright, exhale fully, then inhale through the device with maximum effort against the resistance
  3. 3.Perform 30 breaths through the device, twice daily (morning and evening) — this is the standard POWERbreathe protocol; each session takes about 3-5 minutes
  4. 4.Increase resistance by 1-2 levels each week as the exercises become easier, working toward 50-80% MIP; the key is that each breath should feel effortful but completable
  5. 5.Alternative high-intensity protocol: 5 sets of 6 breaths at 50%+ MIP, once daily, with 1-2 minutes rest between sets; retest MIP every 4 weeks to adjust training load
