What is Habit Reversal Training (HRT)?
A structured behavioral therapy for body-focused repetitive behaviors (BFRBs) including nail biting, hair pulling (trichotillomania), skin picking, thumb sucking, and tic disorders. Developed by Azrin and Nutt in 1973, it remains the gold-standard treatment with strong clinical evidence. The core mechanism: build awareness of the behavior, then train a physically incompatible replacement response.

How do you do Habit Reversal Training (HRT)?
- 1.Awareness Training — become hyper-aware of your habit. For 7 days, log every instance: what were you doing, what triggered it (boredom? stress? watching TV? reading?), which hand/fingers were involved, what was the earliest warning sign (hand moving toward mouth, touching hair, rubbing skin). Ask a trusted person to gently point out when you do it unconsciously. Most people do these behaviors 30-70% without conscious awareness
- 2.Identify your early warning chain: there's always a sequence before the behavior. Example for nail biting: stress → hand moves to face → finger touches lip → teeth find rough edge → biting begins. The earlier you catch the chain, the easier it is to interrupt. Your goal is to catch it at 'hand moves to face,' not at 'biting begins'
- 3.Develop a competing response: choose a physical action that makes the habit impossible and can be sustained for 2-3 minutes. For nail biting: make a fist with both hands for 2 minutes. For hair pulling: grip the arms of your chair or put hands flat on your thighs. For skin picking: hold a smooth stone or squeeze a stress ball. The response must be discreet enough to do in public
- 4.Practice the competing response every time you catch yourself: at the urge, at the early warning sign, or even after you've already started the behavior (stop immediately and switch). Hold the competing response for a full 2-3 minutes or until the urge passes. Initially this feels exhausting — that's normal. After 2-3 weeks of consistent practice, the new response begins to replace the old habit automatically
- 5.Enlist social support: ask 1-2 trusted people (partner, close friend, family member) to remind you when they see the behavior and praise you when they see you use the competing response. Do NOT ask them to punish or shame you — positive reinforcement is critical. Track your progress: count daily occurrences and plot the trend. Most people see a 50-80% reduction within 4-6 weeks
